YES!!! You do! This will prevent one from occurring. My son is 1 week from becoming a year old and hasn't even had a hint of a diaper rash because I apply cream EVERYTIME I change him. I have used Sudocrem on him since he was born, on the advice of several nurses and my pediatrician. It is a wonderful cream, spreads very smoothly, smells nice, etc. References :

The fact that a baby doesn't have nappy rash doesn't mean it will not get baby rash. When conditions are ripe it will happen.

I've never used any creams and my son has never had a rash down there! Still two unopened bottles!
I wouldn't waste your money on it, and chances are if you're not letting him sit in a wet diaper all the time, he won't even get a rash!
